Sometimes seen as a harbinger of doom, the most well-known sighting of the Mothman occurred in Point Pleasant, WV.

Roger and Linda Scarberry and their friends Steve and Mary Mallette went on a drive late at night on November 15, 1966, near the location of an old World War II instillation in the current-day McClintic Wildlife Management Area, near Point Pleasant. Along the road they witnessed a large figure standing near the side of the road, which they described as looking like a man, but having large red eyes and white wings. Frightened by the creature's strange appearance, they sped off towards town, with the creature flying after them. It chased them as far as the city limits before flying back off into the night. The two couples, not really understanding what they saw, claimed they returned to the spot only to be chased by the creature again, this time going to the authorities to report what they had seen. Their story was published in the local newspaper, the Point Pleasant Register, the very next day with the headline "Couples See Man-Sized Bird ... Creature ... Something".

Over the next few weeks more reports about a strange human-like creature with large wings and red eyes were made in the Point Pleasant area. Some people tried to explain away the sightings as birds, such as herons, cranes or owls, but that didn't stop the sightings. Newspapers dubbed the creature the "Mothman", and reported sightings continued to come in through the rest of the year and well into the next.

Then, on December 15, 1967, the Silver Bridge, which connected Point Pleasant to Gallipolis, Ohio via Route 35, collapsed during rush-hour. 46 people were killed in the collapse, with two of the victims never being recovered. While the Mothman and the Silver Bridge collapse were seemingly unrelated events, the sightings of the Mothman seemed to stop right after the bridge collapse, leading some to speculate that the creature was trying to warn people of the impending disaster. While there hasn't been any sightings of the Mothman in Point Pleasant since that time, there have been many different accounts all over the world that seem to mimic the events of 1966 and 1967.

People who have seen the Mothman typically do not attribute it to having moth-like features, and instead refer to it as humanoid. It stands around seven feet tall, with a wingspan of 10 to 12 feet. Most describe it as not having a head or having its head more towards the center of its chest, with the most noticeable feature being its red eyes that appear to reflect light, or in some cases glow. It is black or brown in color, with light colored wings that are more bird or bat-like than moth-like. People who have gotten a good look at the creature tend to say that they were overcome with fear, terror or dread, some even having mental breakdowns after their encounter.
